Thick Toenails in Seniors: 7 Common Reasons + What a Foot Care Nurse Can Safely Do

If your toenails are getting thicker, harder, or more difficult to trim, you’re not alone. Thick toenails are very common after 50, and they can make walking uncomfortable, cause pressure in shoes, and increase the risk of small injuries.

As a Licensed Practical Nurse (LPN) providing foot care, my focus is safe nail and skin care, prevention, and education. This article is not meant to diagnose a medical condition—rather, it’s here to help you understand common reasons nails change and what you can do next.

Why thick toenails matter (especially after 50)

Thicker nails can: – Catch on socks and tear – Press into shoes and cause pain – Make it harder to keep nails trimmed safely – Increase the risk of skin irritation around the nail

If you have diabetes, poor circulation, or reduced feeling in your feet, thick nails can become more than a nuisance—because small problems can turn into bigger ones faster.

7 common reasons toenails get thick

You don’t need to “figure out the exact diagnosis” to take smart, safe steps. Here are common contributors I see in practice:

  1. Normal aging changes
  2. Nails often grow more slowly and can become thicker or more brittle over time.
  3. Pressure from footwear
  4. Tight toe boxes, winter boots, or shoes that rub can repeatedly irritate the nail.
  5. Past nail injury (even years ago)
  6. A stubbed toe, dropped object, or sports injury can change how a nail grows long-term.
  7. Reduced circulation
  8. When blood flow isn’t ideal, nails and skin may change in texture and growth.
  9. Difficulty trimming regularly
  10. If it’s hard to reach your feet or see clearly, nails can become overgrown and harder to manage.
  11. Moisture and sweaty socks
  12. Warm, damp environments can irritate the skin and nails and make problems more persistent.
  13. Long-term health conditions that affect feet
  14. Diabetes and other chronic conditions can affect sensation, circulation, and skin integrity.

Safer at-home nail care tips (for lower-risk adults)

If you do not have diabetes, significant circulation issues, or reduced sensation, these tips may help:

  • Trim after bathing when nails are a bit softer.
  • Use good lighting and sit in a stable chair.
  • Trim straight across (avoid digging down the sides).
  • Use a file to smooth rough edges so nails don’t snag.
  • Choose shoes with a wide toe box to reduce pressure.

A quick safety check

Skip DIY trimming and book professional care if: – Your hands shake or grip is weak – Your eyesight makes it hard to see nail edges – You’ve had bleeding or infections before – You have diabetes, neuropathy, or circulation concerns

What NOT to do (common mistakes)

These are the most common “well-intended” steps that can backfire: – Don’t cut deep into corners to try to prevent an ingrown nail. – Don’t use sharp blades or “corn cutters” on nails or skin. – Don’t aggressively thin a nail with tools you’re not trained to use. – Don’t ignore warning signs like redness, swelling, drainage, warmth, or worsening pain.

When to see a foot care nurse (and when to seek urgent medical attention)

Book a foot care visit if you notice: – Nails are too hard or thick to trim safely – Pain or pressure in shoes – Skin irritation around the nail – You have diabetes, poor circulation, or reduced sensation

Seek urgent medical care if you have: – Spreading redness, warmth, swelling – Drainage, pus, or a bad smell – Fever or feeling unwell – A wound that isn’t improving

What a foot care nurse can safely do

During a mobile foot care visit, I can: – Provide safe trimming and reduction of thick nails – Assess skin for pressure points, irritation, and breakdown risk – Provide education on footwear, socks, and daily foot checks – Help you create a simple plan to prevent recurrence
